Effects of Mulligan’s Mobilization Adjunct to Agility and Perturbation Exercises in Subjects with Knee Osteoarthritis

Background: Osteoarthritis of the knee is one of the common diseases, mainly affecting the articular cartilage of a synovial joint. Which results in pain, decreased proprioception, muscle weakness, and disability. Mulligan’s mobilization helps in relieving pain and improves ROM, but lacks in stability. Agility and perturbation exercises improve stability and proprioception but lack in the improvement of ROM. Mulligan’s mobilization and Agility and Perturbation exercises in combination can bring better outcome results. Purpose: The purpose of the study is to find out whether Mulligan’s mobilization MWM (Internal rotation glide) as an adjunct to Agility and Perturbation can bring better outcome in patients when compared to Mulligan’s alone. Methods: 60 patients were assigned to two groups 30 each group and subjects were selected by convenient sampling method. Group A with mulligan’s mobilization alone and group B SWD with Mulligan’s mobilization as an adjunct to Agility and Perturbation exercises. The treatment session was performed on alternate days for 6weeks. Each subject performed all the measurement with knee function assessed NPRS, knee ROM by Goniometer, Timed up and go scale (TUG) and WOMAC index. All outcome were tested at starting the day, week 3 and final week 6 of pre and post-treatment. Results: The group analysis was done for NPRS, TUG, ROM, WOMAC in the evaluation of knee function was done using independent ‘t’ test. Within the groups paired t-test used and showed statistically significant. Conclusion: It is concluded that Mulligan’s technique with Agility and perturbation exercises is a better treatment approach than Mulligan’s with conventional exercises.

Implant Maintenance-A New Protocol

As implant treatment becomes part of mainstream dental therapy, dental offices should implement protocols for individualized, systematic and continuous supportive care of the peri-implant tissues. This short communication article suggests guidelines for maintenance care of dental implants. The preliminary assessment should begin with updating the patient’s medical and dental histories. The clinical implant should be examined to evaluate the following: the condition of the soft tissues, plaque index, clinical probing depth, bleeding on probing, suppuration, the stability of soft-tissue margins, keratinized tissue, mobility, and occlusion. If the clinical signs suggest the presence of peri-implantitis, radiography of the site is advisable, to confirm the diagnosis. Appropriate treatment should be pursued according to any diagnosis reached during the examination, including (but not limited to) instructions on oral hygiene, removal of supra- and sub-gingival plaque and calculus, occlusal adjustment, relining of a removable prosthesis or surgery.

Novel Protocol to Manage Peri-implantitis

Peri-implantitis is characterized by an inflammatory reaction that affects the hard and soft tissue, which results in loss of supporting bone and pocket formation surrounding the functioning osseointegrated implant. This short communication proposes new protocols for management of periimplantisis. The non-surgical approach involves the mechanical surface debridement using carbon or titanium currettes, laser light, and antibiotics whereas, the surgical approach involves implantoplasty, elevation of mucoperiosteal flap and removal of peri-inflammatory granulation tissue followed by surface decontamination and bone grafting.
